Note:

1. This declaration document is used for the approval and filing of the department, in which there must be at least three originals for “Original” to handle other procedures.

2. Relevant transfer declaration documents shall be issued by the town or subdistrict for the enterprises registered in the town or subdistrict.

3. Three originals shall be prepared for this declaration document, one original and one copy shall be firstly submitted (two originals for the application report).
